Undated Daily - Chronic Pain Tracking Printable

A daily insert I created to keep track of my health when my chronic pain was at its worst. I wanted to ensure my mental and physical health were monitored but I did not want a bulky spread that took up multiple pages. This version was created in 2022 and curated in 2024. 

Included are sticky notes to help remind you what certain abbreviations are and an example of how I use my tracker.
